Initial Assessment Enquiry & Referral Form

This short form is the crucial first step. Once submitted, Jo will review the details and send the required Family and School Questionnaires to complete your child's referral - an assessment cannot take place with out them. This form is used by all parties - Families/Carers, Schools/SENCOs and Local Authorities, however only schools and LA's can request a Sample Redacted Report.

Thank you for taking this important first step. Jo will personally review your enquiry and get in touch to outline the next steps. This will include sending you the essential Family and School Questionnaires. These forms must be completed, and the assessment fee must be paid, before your child's specialist assessment can be formally scheduled and secured.
