The Referral Process This form is the first step in our specialist workflow. Once submitted, I will personally review the details and send out the necessary questionnaires. These documents are essential to provide the 360° view required for a defensible assessment and robust report.

Note: This form is for use by Families, Schools and Local Authorities. Schools and LAs may also use this form to request a Sample Redacted Report.

Thank you for taking this important first step. Jo will personally review your enquiry and get in touch to outline the next steps. This will include sending you the essential Family and School Questionnaires. These forms must be completed, and the assessment fee must be paid before your child's specialist assessment can be formally scheduled and secured.
